Starting an Assignment
Below you will find all the necessary forms to enroll in our benefits program. Employees on full-time assignments (30 hours a week or more) are eligible for the following benefits as soon as they start work:
Paid Time-Off
To promote work-life balance, we provide paid time off to our contract employees who are on PTO-eligible assignments. PTO begins accruing at the beginning of the assignment and can be used after 30 days. Assuming you work full-time hours, you can earn 8.5 paid days off per year. After 60 Days On Assignment
After 60 days on assignment, full-time employees are eligible to enroll in Celarity’s insurance plans and participate in our 401(k) plan.
Health Insurance Plans
Our health plans include separate coverage for health and dental. Vision is covered under the health plan, excluding glasses and contacts. Celarity covers up to 60% of your health insurance premiums. Invest in yourself and take advantage of Celarity’s retirement savings plan benefit.
Follow this link to access machine-readable files (MRFs) that are made available in response to federal Transparency in Coverage rules. The MRFs contain pricing information about covered items and services and are formatted to allow third parties to access and analyze the data.
After One Year on Assignment
401(K) Contribution Match
After one year and at least 1000 hours on assignment, employees are eligible for a Celarity match of 100% of your deferrals up to the first 3% of your compensation and 50% of your deferrals on the next 2% of your compensation.
After Assignment Ends
If your assignment with Celarity ends, your eligible benefits terminate at the end of the month. If you are placed in another position within 13 weeks of your previous termination date, your benefits may be reinstated on the first of the month following your reinstatement date.
After a consecutive break of 13 weeks or more, you are considered a rehire on the start date of your next placement. A rehire is subject to the new hire waiting periods for all benefits.
